Molecular Difference: Chemistry Lesson Begins
Before making a decision, we need to understand what these materials are. They are both biodegradable (dissolving) and biocompatible (safe) polymers.
- PLLA (Polylactic Acid): It is a synthetic substance derived from milk acid. It is actually a powdered form of dissolving surgical threads. It is given under the skin as a liquid, it does not give volume, it only stimulates.
- PCL (Polycaprolactone): It is a soft biopolymer used in medical implants. PCL microspheres float in a carrier gel (CMC), creating a physical volume (filling effect) as soon as they are injected.
Big Comparison: Speed, Volume and Persistence

Which Face Type Should Choose Which One?
This decision depends on what your face needs. There is no “best product”, there is a “product that suits you”.
Scenario 1: “My face is very thin, my temples are sunken.”
Winner: PLLA (Sculptra).
On thin faces, PLLA provides a very natural fullness by gently thickening the skin. PCL can sometimes look too harsh or voluminous on these faces. The “gradual” effect of PLLA gives a more controllable result on thin faces.
Scenario 2: “I have no cheekbones, my chin is back.”
Winner: PCL (Smart Fill).
PCL is superior to mimic bone structure and provide projection. It gives instant shape thanks to its gel structure. PLLA cannot create a hard line like bone, it gives more soft tissue volume.
Scenario 3: “Let no one understand, let me change slowly.”
Winner: PLLA.
If you don't want to change in a day, the 3-month effect of PLLA is for you. When you have PCL, the change is noticeable the moment you leave the clinic.
Safety and Side Effects: Which is Safer?
Both products are FDA and CE approved. But there are nuances in their safety profiles:
- Nodule Risk: Without massage (5-5-5 rule), the risk of clumping is slightly higher with PLLA than with PCL. No massage is required with PCL.
- Return Neither has a melting enzyme. So if you don't like it, you cannot melt it. However, since the effect of PLLA occurs slowly, it is easier to see the progress in the process (between sessions) and intervene. PCL is done and finished in one go.

Frequently Asked Questions (FAQ)
PCL is a slightly harder material than PLLA. However, when it is performed at the right depth (over bone or deep dermis), no external stiffness is felt, only a tighter tissue.
It is not recommended to mix in the same area in the same session. However, 6 months after PLLA treatment (when collagen production is completed), PCL can be performed for the missing volumes. It can be performed in different areas in the same session.
PCL (Smart Filler) may seem more expensive as a single session price. However, since PLLA usually requires 2-3 sessions, the total treatment costs are close to each other. When divided by the duration of permanence (2-4 years), both are more economical than classical fillers.
The forehead is a dangerous area. PCL (Smart Filler) can be used as a forehead filler in careful hands. PLLA, on the other hand, is not usually applied to the forehead (due to the risk of nodules). The safest option for the forehead is still Hyaluronic Acid filler.
